Eligibility

Who is eligible for benefits?
You are eligible for benefits if you are an active, full-time employee working an average of 30 hours per week.
When are new employees eligible for benefits?
Employees are eligible for benefits on the first of the month following their start date. For example, if an employee starts any day in August, their coverage will be effective September 1.

How do I enroll?

Who is an eligible dependent?
    • Your legal spouse
    • Children up to age 26 (regardless of student status)
    • Children who are fully dependent on you for support due to a mental or physical disability and who are indicated as such on your federal return; coverage may continue past age 26
    • Domestic Partners – contact your HR department to see if they qualify

     An eligible child is defined as follows: a natural child, a step-child, a legally adopted child, a child for whom you have legal guardianship over, or a disabled child incapable of self-sustaining employment enrolled prior to age 26.

What is a qualifying life event?
A qualifying life event is an event in your life that makes you eligible for a special enrollment period.
What is considered a qualifying life event?
    • Marriage, divorce, legal separation, or annulment
    • Birth, adoption, or placement for adoption of an eligible child
    • Death of a spouse or child
    • Change in your spouse’s employment that affects benefits eligibility
    • Change in your child’s eligibility for benefits (reaching the age limit)
    • Change in residence that affects your eligibility for coverage
    • Significant change in coverage or cost to your own, your spouse’s. or your child’s benefit plans
    • FMLA Leave, COBRA event, Court Judgement or Decree
    • Becoming eligible for Medicare or Medicaid
    • Receiving a Qualified Medical Child Support Order

    If you have a Qualifying Life Event, and you want to request a mid-year change, you must notify Human Resources and complete your election changes within 31 days following the event. Be prepared to provide documentation to support the Qualifying Life Event.

How long do I have to submit a qualifying life event?
When you have a qualifying life event, you can make updates to your benefits coverage by submitting proof of the qualifying life event within 30 days of the event.
When is open enrollment?

During the month of July for medical and March for dental and vision.

How can I find a network provider for my Primary Coverage?

Go to Find a Doctor at www.cigna.com.

How do I get my insurance card?
  • By mail: After you enroll, you should receive an ID card in the mail; check with HR to make sure they have your correct mailing address on file. If an address correction needs to be made, be sure to let HR know.
  • Printing temporary ID cards by visiting www.abadmin.com.
What is the group number for my Primary Medical Coverage?

How To File Claims

How do I submit my medical claims?

We have set up an electronic payer ID for your provider to file the claim electronically; let your Provider file the claim for the Major Medical plan (Primary) and your Secondary Plan.

The insured will need to provide his/her primary medical card as well as the Secondary Card to the provider. Most providers will file the claim electronically because it is easy and they get paid faster. A claim contact phone number is printed on the ID card along with the claim’s payer information for the provider. If needed, our customer service staff can and will send a letter to each provider explaining how to file claims.

The Provider Claims Guide is a helpful tool to explain to the provider the appropriate steps to file the secondary claim.  Share a copy of this guide with your provider to help make sure your claims are paid in a timely manner.

Who is eligible for the 401(K) Plan?

All employees 18 and older.

When are new employees eligible to enroll in the 401(K) Plan?

Employees are eligible to enroll in the 401(K) Plan after completing 90 days of employment.

How much can I contribute to the plan?

You can contribute up to the maximum allowed by the IRS, for 2020 the dollar limit is $19,500.
